$125 per 45-minute session
Insurance
Services may be covered in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health benefits?
- How much does my plan cover for an out-of-network provider?
- What is the deductible and has it been met?
- How many sessions per calendar year does my plan cover?
- What is the coverage amount per therapy session?
- Is pre-authorization required?
- Will you pay for biofeedback for (your symptom) and what code is it covered under (90901;90812)?
Payment
Cash or check accepted.
Cancellation Policy
All canceled appointments without 24 hours notice will be charged at the usual rate. This is payable at the next session.
